It is the goal of the Judiciary to serve you in a manner that is fair, transparent, prompt and respectful.
Please let us know if we are succeeding or how we may improve by filling out the following survey. Your responses are important in helping us improve our services to you and to our community.

This process applies to court users, unless otherwise specified, and is not intended to address issues or complaints that are subject to other legal, statutory and/or administrative processes, and/or matters subject to collective bargaining grievance procedures. Any complaint that is misfiled under this process shall not substitute as a proper filing under the applicable complaint/appeal procedure, and shall not preserve timeliness.

A Note On Privacy
This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven’t) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
